The solution is to record and capture everything that is said in the meeting.  This can be done by digitally recording what is being said to a PC.  At the same time, the note taker can continue to write down the main points of what is being said.  The notes are cross referenced to the recorded conversations in real time.  This means that when the minutes are written down later on the note taker can click on their main point and refer back to what was actually said at the time and not what they can recall.  The minutes can be distributed electronically along with a recording of the entire meeting to all parties.  There is no longer a dispute on what was said, because everyone has a summary written along with the actual meeting.  If a dispute were to arise then one click on the minute point will retrieve the verbal conversation.  There is now clarity in what was actually said and decided.
